How to: Open a blank form.
Solution:
In the Database window, click the 'Forms' tab. Click 'New'. Select 'Design View' and choose 'OK'.
1) If the Database window is not activated, activate the Database window.
2) Click the 'Forms' tab in the Database window.

3) Click 'New'. (The New Form dialog box appears.)
4) Select 'Design View' from the list.
NOTE: Do not specify a table or query in the 'Choose a table or query' list.
5) Click 'OK'. (A blank form appears.)
